This 23 year old woman has had several previous breast implant procedures. She presents with severe bilateral periprosthetic capsular contracture (Baker grade 4) as well as abnormally wide cleavage and severe waviness and rippling of the breasts. Her implants are in the subglandular position. The corrective surgery consisted of removal of the implants, conversion to a subpectoral pocket and insertion of the same implants in the new submuscular pocket. At the same time, the pocket was reconfigured to give the patient improved cleavage, her postoperative result with noticeable improvement in symmetry, correction of capsular contracture, improvement of the cleavage and correction of waviness and rippling is shown at six months.
