Notice of Privacy Practices

Acknowledgment of receipt

Neal Handel, M.D., F.A.C.S.

The Health Insurance Portability & Accountability Act of 1996 (“HIPAA”) is a Federal program that requests that all medical records and other individually identifiable health information used or disclosed by us in any form, whether electronically, on paper, or orally are kept properly confidential. This Act gives you, the patient, the right to understand and control how your protected health information (“PHI”) is used. HIPAA provides penalties for covered entities that misuse personal health information.

As required by HIPAA, we prepared this explanation of how we are to maintain the privacy of your health information and how we may disclose your personal information.

We may use and disclose your medical records only for each of the following purposes: treatment, payment and health care operation.

  • Treatment means providing, coordinating, or managing health care and related services by one or more healthcare providers. An example of this is a primary care doctor referring you to a specialist doctor.
  • Payment means such activities as obtaining reimbursement for services, confirming coverage, billing or collections activities, and utilization review. An example of this would include sending your insurance company a bill for your visit and/or verifying coverage prior to a surgery.
  • Health Care Operations include business aspects of running our practice, such as conducting quality assessments and improving activities, auditing functions, cost management analysis, and customer service. An example of this would be new patient survey cards.
  • The practice may also be required or permitted to disclose your PHI for law enforcement and other legitimate reasons. In all situations, we shall do our best to assure its continued confidentiality to the extent possible.

We may also create and distribute de-identified health information by removing all reference to individually identifiable information. We may contact you, by phone or in writing, to provide appointment reminders or information about treatment alternatives or other health-related benefits and services, in addition to other fundraising communications, that may be of interest to you. You do have the right to “opt out” with respect to receiving fundraising communications from us.

The following use and disclosures of PHI will only be made pursuant to us receiving a written authorization from you:

  • Most uses and disclosure of psychotherapy notes;
  • Uses and disclosure of your PHI for marketing purposes, including subsidized treatment and health care operations;
  • Disclosures that constitute a sale of PHI under HIPAA; and
  • Other uses and disclosures not described in this notice.

You may revoke such authorization in writing and we are required to honor and abide by that written request, except to the extent that we have already taken actions relying on your prior authorization.

You may have the following rights with respect to your PHI.

    • ·  The right to request restrictions on certain uses and disclosures of PHI, including those related to disclosures of family members, other relatives, close personal friends, or any other person identified by you. We are, however, not required to honor a request restriction except in limited circumstances, which we shall explain if you ask. If we do agree to the restriction, we must abide by it unless you agree in writing to remove it.
    • ·  The right to reasonable requests to receive confidential communications of Protected Health Information by alternative means or at alternative locations.
    • ·  The right to inspect and copy your PHI.
    • ·  The right to amend your PHI.
    • ·  The right to receive an accounting of disclosures of your PHI.
    • ·  The right to obtain a paper copy of this notice from us upon request.
    • ·  The right to be advised of your unprotected PHI is intentionally or unintentionally disclosed.

If you have paid for services “out of pocket,” in full and in advance, and you request that we not disclose PHI related solely to those services to a health plan, we will accommodate your request, except where we are required by law to make a disclosure.

We are required by law to maintain the privacy of your Protected Health Information and to provide you the notice of our legal duties and our privacy practice with respect to PHI.

This notice if effective as of ________________________ and it is our intention to abide by the terms of the Notice of Privacy Practices and HIPAA Regulations currently in effect. We reserve the right to change the terms of our Notice of Privacy Practice and to make the new notice provision effective for all PHI that we maintain. We will post and you may request a written copy of the revised Notice of Privacy Practice from our office.

You have recourse if you feel that your protections have been violated by our office. You have the right to file a formal, written complaint with office and with the Department of Health and Human Services, Office of Civil Rights. We will not retaliate against you for filing a complaint.

For more information feel free to contact the Practice Compliance Officer Nina B Floric at 805.682.9100, in person or in writing.

HIPPA Privacy Policy

The Department of Health and Human Services, Office of Civil Rights, under the Public Law 104-191, (The Health Insurance Portability and Accountability Act of 1996) (HIPAA), mandates that we issue this new revised Privacy Notice to our patients. This notice to our patients meets all current requirements as it relates to Standards for Privacy of Individually Identifiable Health Information IIHI); affecting our patients. You are urged to read this notice.

As part of the Privacy Standard, implemented on April 14, 2001, you are required to provide this office with a new, signed and dated, Consent Agreement. Every patient must receive our new Privacy Notice and execute a new Consent Agreement before this office may use your information for treatment, payment, or other health care operations (TPO).

Our Privacy Notice informs you of our use and disclosure of your Protected Health Information (PHI), defined as: “any information, whether oral or recorded in any medium, that is either created or received by a health care provider, health plan, public health authority, employer, life insurance company, school or university or clearinghouse and that relates to the past, present or future physical or mental health or condition of an individual, the provision of health care to an individual, or the past present or future payment for the provision of health care to an individual.”

Our office will use or disclose your PHI for purposes of treatment, payment and other healthcare purposes as required to provide you the best quality healthcare services that we offer to the extent permitted by your Consent Agreement or in such specific situations, by your signed and dated Authorization. It is our policy to control access to your PHI; and even in cases where access is permitted, we exercise a “minimum necessary information” restriction to that access. We define the minimum necessary information as the minimum necessary to accomplish the intent of the request.

An Authorization differs from a Consent Agreement in that it is very specific with regard to the information allowed to be disclosed or used, the individual or entity to which the information may be disclosed to, the intent for which it may be disclosed, and the date that it was initiated which may include the duration of the authorization. This is a form, separate from the Consent Agreement, and usually used only for one specific request for information. In the event of a non-healthcare related request for personal health information this office will request you to complete an Authorization Form.

You, as our patient, may revoke any Consent Agreement or Authorization at any time and all use and disclosure and administration of related healthcare services will be revised accordingly, with the exception of matters already in process as a result of prior use of your PHI. To revoke either the Consent Agreement or the Authorization you will have to provide this office with a written request with your signature and date and your specific instructions regarding an existing Authorization or Consent Agreement. Any revocation will not apply to information already used or disclosed.

If you had a “personal representative” initiate as Authorization you may revoke that authorization at any time.

You, the patient have access to your health care information and may request to examine your information, may request copies of your information, and under the law you may request amendments to your information. The physician or principal will exercise professional judgment with regard to requests for amendments and is not bound by law to make any changes to the information. If the physician or professional agrees with the request to amend the information, we are bound by law to abide by the changes.

In limited circumstances, The Privacy Standard permits, but does not require, covered entities to continue certain existing disclosures of health information without individual authorization for specific public responsibilities.

These permitted disclosures include: emergency circumstances; identification of the body of a deceased person, or to assist in determining the cause of death; public health needs; research, generally limited to when a waiver of authorization is independently approved by a privacy board or Institutional Review Board; oversight of the health care system; judicial and administrative proceedings; limited law enforcement activities; and activities related to national defense and security. There are specific state laws that required the disclosure of health care information related to Hepatitis C, and AIDS. Where the state laws are more stringent than HIPAA Privacy Standard, the state laws will prevail.

All of these disclosures could occur previously under former laws and regulations however; The Privacy Standard establishes new safeguards and limits. If there is no other law requiring that your information be disclosed, we will use our professional judgments to decide whether to disclose any information, reflecting our own policies and ethical principles.

On some occasions we may furnish your PHI to a third party. This could be an insurance company for the purpose of payment or another health care provider for further treatment or additional services. Although we will institute a “chain of trust” contract and monitor our business associates’ contracts with us, we cannot absolutely guarantee that they will not use or disclose your PHI in such a way as to violate the Privacy Standard.

Although the law requires a signed and dated Privacy Notice, this office does not demand that you sign this agreement as a condition of receiving care. It is the law that your rights are communicated in this manner.

It is our practice to retain information about non-healthcare related requests for your health care information for a period of six years.

In complying with the Privacy Standard, we have appointed a Privacy Officer, trained our Privacy Officer and the staff in the law, and implemented policies to protect your PHI. We have instituted privacy and security processes to guard and protect your IIHI. This office is taking and continues to monitor and improve steps for the protection of your information and to remain in compliance with the law.

Facelift Plastic Surgery Cost

Facelift Plastic Surgery Cost

Cost is as important as the doctor’s credentials while considering facelift surgery. The key to looking youthful is cosmetic surgery procedures like a facelift which erases signs of aging like wrinkles, sagging skin etc. to enhance physical appearance. Premature aging is triggered by environmental factors, unhealthy lifestyle habits and poor diet. Who wants their skin looking dull like an 80 year old when they are in their mid 40’s? The science behind age reversal isn’t discovered yet but superficial age defying techniques are in demand.

Rhytidectomy or a face lift is performed by a plastic surgeon like Dr. Neal Handel, by tightening and lifting the underlying facial muscles to incorporate sharp contouring typical of youth, back into the face. Fat pockets are also removed. Facelift surgery is nothing short of magic, you walk in to the clinic looking older than you actually are and you walk out looking 20 years younger. The detailed artist contouring is achieved only by an exemplary artistic vision. A cosmetic surgeon’s job requires special trained skill sets which highly influences the end cost of the surgery. There is a saying that a thing of beauty lasts forever, so why not beautify the first noticed appearance: the face.

Cost Factors

Doctors cost

Doctor’s experience determines if the cost. More the number of satisfied patients greater the doctor’s fee. Cosmetic surgery handled by an exclusive plastic surgeon with face lift as their area of forte generally is priced higher than the surgery over seen by a regular surgeon who also takes on cases of plastic surgery. The location of the clinic also decides the cost. Consider a clinic in a big metropolitan city where the cost of running an establishment is pricy; to compensate money spent on the clinic’s sustenance, cost of the procedure is increased. Face lifts in smaller cities are more within a set budget. A recent statistics revealed the average surgeon’s cost is $7500.

Minimal invasive surgery with insignificant scaring using current surgery technology and equipment are priced higher. The end result is naturally younger looking skin with less recovery time. Extensive surgery like superficial masculoaponeurotic system (SAMS) and deep plane facelift cost more than minima invasive surgery like short scar facelift.

Surgery cost

The added cost of administering anesthesia, the anesthesiologist’s cost, the operating room charges, medication cost, hospital fees and other similar expenses are chargeable on the patient. Anesthesia fees amount to $1000-$1300 while the hospital fee is $500-$2000.

Insurance coverage

Few policies cover cosmetic surgery. Policy covered procedures cost less upfront than those which require full payment.

Combined procedures

If you opt for a face reconstructive surgery like rhinoplasty along with a face lift the total cost will be the sum of all the individual surgeries. It will work out cheaper if you get the surgeries at once than spacing them out and undergoing individual procedures.

Facelift Plastic Surgery Overview

Facelift Plastic Surgery Overview

The search for the fountain of youth is eternal. There is no mythical elixir that reverses aging. Science bestows the ability to discover or develop new technologies that can turn farfetched ideas into reality. Aging can be reversed at least superficially by a plastic surgery procedure popularly known as facelift.

The most effective proven way to remove skin dullness, sagging and other signs of aging is by a facelift. Medically known as Rhytidectomy the procedure involves lifting off the primary layer of the skin to tighten the tissue underneath followed rolling the skin layer back on evenly. For the procedure you are sedated either by administering general anesthesia intravenously or local anesthesia to freeze sensation around specific regions. The next step is the incision that starts at the temple and ends near the chin in front of the ear to enable lifting of the skin to tighten the underlying tissues. Either fat is removed or the extra skin. The skin is then reset and the incision is closed by sutures. The scars of the surgery aren’t prominent because the location of the incision is at the place where the skin would naturally fold. Another variant is the limited access facelift where the incision is made either over the upper-lip or into the lower eye lid. Smaller incisions that are closer to the ear are also made. The cosmetic procedure extends to the neck and the chin to remove loose and sagging skin. For such procedures the cut starts at the front of the lower ear lobe and extends to the base of the hair line. Time taken to complete one facelift is several hours. Although you can go home after the surgery many prefer to stay overnight at the medical care facility.

The results vary from person to person. For some the desired outcome is achieved by procedures that are done through non-invasive surgery like chemical peels. For some a brow lift or a neck lift might be sufficient. By having realistic expectations the end results will be more satisfactory.

The procedure is not suitable for those whose:

• Skin has lost its elasticity with minimized flexibility and suppleness.

• The bone structure of the face is not well defined and therefore doesn’t not offer the required support framework.

• A history of serious medical conditions that interfere with the process like a bleeding disorder.

Dr. Neal Handel explains that all surgeries have their own associated risks. The possible negative repercussions of a facelift are infection, uncontrolled hair loss, scarring, blood clots that might reach the lungs and heart (pulmonary embolism), allergic reaction to anesthesia, possible damage to the facial nerves that might translate to a spasm or face paralysis and bleeding below the skin.
The benefits of the procedure are exploited by men too. The procedure for both sexes is the same. Facelifts are beneficial for senior citizens who want to wipe off a certain number off their age.

Neal Handel MD is the first practice to introduce FDA approved Lipogems to the United States!

What is Lipogems?

Lipogems refers to the method of processing and purifying fat for use as an injectable filler that has powerful rejuvenation and anti ageing properties.

Lipogems system ensures that the fat processed is rich in stem cells and growth factors that can effectively restore volume, tone and texture in the face and hands.

The process is completed during a single session of approximately 90 minutes.

Dr. Neal Handel Introduces Lipogems

Dr. Handel’s office is the first plastic surgery practice in the US to offer Lipogems, which is an FDA-approved cosmetic rejuvenation procedure.

Unlike traditional cosmetic surgery, which will involve a prolonged process from preparation to recovery, Lipogems is a less invasive procedure with minimal downtime. Dr. Handel provides this procedure to his patients in Santa Barbara, CA and Beverly Hills, CA.

Lipogems does not require the addition of any enzymes or chemicals before the fat can be re-injected into the targeted areas, this ensures that the re-injected fat comprises only the patient’s own refined cells. The minimal gap between fat extraction and injection cuts down the risk of contamination and avoids the need to add preservative chemicals.

Lipogems device is entirely disposable after single use, which ensures sterility. The fat process with the Lipogems system has a higher graft survival rate, which will improve the quality of surrounding tissue and the appearance of the overlying skin. Unlike other fat transfer techniques, with the Lipogems injection process, the processed fat flows smoothly, which helps to reduce discomfort during and after the procedure.

Areas suited to Lipogems rejuvenation include:

  • Mid face
  • Tear troughs
  • Nasolabial folds
  • Lips and around the mouth
  • The back of hands

Plastic Surgery Procedures Offered

Plastic Surgery

Dr. Handel Plastic SurgeonWith over 38 years of experience in the field of plastic, reconstructive, and cosmetic surgery, Dr. Neal Handel is one of the leading figures in the field. Dr. Handel provides a wide range of surgical procedures for the breast, body, and face. He focuses on customizing the surgery according to the unique anatomical and aesthetic needs of a patient.

Dr. Handel’s state of the art practice is equipped with cutting edge surgical technologies to deliver safe, predictable, and sustainable outcomes. As a well-known plastic surgeon in the region, Dr. Handel receives patients from Beverly Hills, California, and surrounding communities for a wide variety of surgical and non-surgical procedures.

Breast Procedures

The major breast cosmetic surgery procedures provided at Dr. Handel’s practice include breast augmentation, breast lift, breast lift with augmentation, breast reduction, breast reconstruction, male breast reduction or gynecomastia, breast asymmetry correction with implants or fat grafting, and breast revision surgery.

Breast augmentation and breast lift are major aesthetic procedures that are sought by a large number of women to achieve a more attractive breast appearance. In some cases, the surgeon may combine breast augmentation and breast lift to achieve more comprehensive results and to meet the patient’s personal aesthetic goals.

Procedures such as breast reduction and gynecomastia may be performed as medically necessary procedures or for cosmetic reasons. Breast revision is relatively more complex as compared to the primary breast implant surgery. This procedure may be performed if the patient develops capsular contracture or another complication with the implants.

Face Procedures

As a leading plastic surgeon in Santa Barbara, California, Dr. Handel provides a wide range of facial surgery procedures, including brow lift, eyelid lift, facelift, neck lift, chin implant, otoplasty, rhinoplasty, fat transfer, and cheek implant surgery. Sometimes two or more procedures may be performed in conjunction to achieve more effective outcomes.

Brow lift and eyelid lift are upper face procedures that can help to rejuvenate the appearance of the eyes and the forehead. Facelift is one of the more popular and comprehensive procedures that can take several years off the patient’s face and restore a more youthful facial appearance. It is usually combined with neck lift to achieve more uniform looking results. Rhinoplasty and chin and cheek implants are also popular procedures designed to enhance the facial look and individual features.

Body Contouring Procedures

Some of the major body contouring procedures, including tummy tuck, arm lift, butt lift, liposuction, fat grafting, and labiaplasty (vagina lip reduction), are performed by Dr. Handel. Tummy tuck or abdominoplasty is a procedure designed to remove excess fat tissue and loose skin from the abdominal area to restore a flatter and firmer tummy.

Liposuction ranks as the most popular plastic surgery procedure in the US today. This targeted fat reduction surgery can be performed using less invasive techniques to achieve highly desirable results. Fat grafting, an arm and butt lift are effective body contouring procedures performed by Dr. Handel. Labiaplasty is a procedure aimed at providing vaginal rejuvenation and a more aesthetically appealing appearance of the vagina.

What is Lipogems?

Lipogems is a cutting edge fat transfer procedure designed to achieve facial rejuvenation and volumizing of certain other areas of the body. The procedure involves processing and purifying fat that can be delivered to the depleted areas of the face as an injectable filler.

Lipogems system ensures that the fat so processed is rich in stem cells and growth factors that can effectively restore volume, tone and texture in the face and hands. Lipogems can be a powerful anti-aging solution for patients who wish to avoid facelift cosmetic surgery, but are keen to achieve a rejuvenated facial look.

Dr. Neal Handel Introduces Lipogems

Leading plastic surgeon Dr. Neal Handel has been providing state of the art cosmetic and reconstructive surgical procedures to patients in Santa Barbara, California and surrounding communities for nearly four decades.

The entire liposuction and re-injection process can be completed in just one session of about 90 minutes.

Differentiated Benefits

Unlike the standard fat transfer procedures that a plastic surgeon may perform, Lipogems does not require the addition of any enzymes or chemicals before the fat can be re-injected into the targeted areas. This ensures that the re-injected fat comprises only the patient’s own refined cells.

Dr. Handel provides this procedure to his patients in Santa Barbara, CA and other areas.

Volume enhancement with Lipogems can occur most significantly in areas such as the mid face and beneath the eyes. The procedure will also vastly improve the elasticity, tone, and vitality of the overlying skin. Most effective results with Lipogems can be achieved in the following areas:

  • Mid face
  • Tear troughs
  • Nasolabial folds
  • Lips and around the mouth
  • Back of the hands

Effects in the Tear Trough Area

Lipogems fat transfer technique can produce dramatic improvements to a patient suffering from hollows beneath the eyes, dark under eye circles, and crepe-like skin that give the appearance of tiredness and aging.

Refined fat helps to concentrate cells, which support the elasticity, soft tissue structure, volume and tone of the skin. The effects are ongoing, sustainable, and will continue to enhance tissue quality and elasticity under the eyes.

Effects in the Mid Face

Sagging skin in the mid face and loss of cheek volume can be addressed very effectively with Lipogems. When the treatment in the mid face is combined with targeted treatment around the eyes, lips and mouth, it can achieve overall enhancement of facial contour and definition. Facial fullness can be restored, giving a rejuvenated and youthful look.

Breast Augmentation Surgery Costs

Breast Augmentation Surgery

Breast Augmentation Plastic Surgery Costs Santa Barbara Breast augmentation is a major cosmetic surgery, which ought to be performed only by an experienced surgeon at a recognized surgical facility. While safety and quality continue to be the undisputed prime factors, but for a number of women, the dollar figure of the procedure still remains a starting point while considering breast augmentation.

Dr. Neal Handel is a leading plastic surgeon with nearly four decades of experience in the field. During the initial consultation process, Dr. Handel will discuss all aspects of the procedure, including its estimated costs. His first goal is to help the patient make an informed choice. Dr. Handel receives patients for breast augmentation from Beverly Hills, California, and surrounding areas.

What is the Average Cost?

As a customizable procedure with cost variations in key components such as breast implant qualities, the overall financial amount of breast augmentation can vary from one patient to another. Several other factors will also influence the cost. In terms of average cost, the national average cost of breast augmentation cosmetic surgery in 2014 in the US was $3,708. This is the figure provided by the American Society of Plastic Surgeons (ASPS).

However, the ASPS clarified that this cost is exclusive of the costs of anesthesia, operating facility costs, and other surgical expenses. When all such dollar figure components as well as additional expenses such as the price tag for prescription medications, medical tests, mammograms, surgical accessories and post-surgical visits are considered, the total cost may range from $7,000 to $14,000.

Where the Practice is Located

Practice location is a major factor in determining the overall monetary amount of breast augmentation. In large cities as well as the ones located along the East and West Coasts, the average dollar figure of living as well as the cost of real estate is usually higher. This will reflect in higher costs of most goods and services, including plastic surgery. Therefore, the financial impact of breast augmentation in Beverly Hills, California may not be the same as the dollar amount in, say, Arizona.

Breast Implants and Your Checking Account

Silicone gel based implants will usually cost about $1,000 higher as compared to traditional saline implants. Some of the latest versions in FDA approved highly cohesive and form stable implants, which are also known as gummy bear implants, may cost about $2,000 to $3,000 higher as compared to saline implants.

Fee Charged by the Surgeon

The plastic surgeon may charge a fee for breast augmentation procedure on the basis of qualifications, experience and reputation in the field of breast surgery. According to the ASPS, it is important to choose the right surgeon for such a major procedure, and factors such as the surgeon’s fee should only be a secondary consideration.

Insurance and Finance

The cost of breast augmentation may be covered by the health insurer only if the procedure is performed as a medical requirement following a mastectomy or as a part of breast reconstruction. Other patients can explore multiple financing options, such as compassionate finance from private medical financing firms. Such finance is usually available at competitive interest rates and easy monthly installments.
